Production began in February 1921 under the working title, Ishmael, as stated by the 5 March 1921 Motion Picture News. The 1 June 1921 Canadian Moving Picture Digest noted that the title character was named after a figure in the Old Testament, known as the exiled illegitimate son of Abraham. Hearts of Youth had since been announced as the film’s official title. The July 1921 Picture-Play erroneously referred to the feature as Youthful Hearts. Additional cast members included Mary Huntress (1921 Motion Picture News Studio Directory) and Philo McCullogh (July 1922 Picturegoer).
Hearts of Youth was released on 8 May 1921, followed by a 13 May 1921 showing at Loew’s New York Theatre in New York City.

Ishmael Worth forsakes his sweetheart, Beatrice Merlin, because he believes himself to be illegitimate and does not want to bring shame to her. His father's previous wife, Hannah, now known as Mrs. Grey, was long believed to be dead. She reappears to confront Ishmael, who learns that she was a bigamist, and therefore never legally married to his father.
